Separation from the clinical portal
This public-facing website is separate from the clinical platform designated by Serendipia Therapy. The public forms are for administrative inquiries and do not replace clinical intake, informed consent, clinical records, or telehealth.
Information we collect
When you use the contact or evaluation-interest forms, we collect the administrative information you provide. This may include your name, email address, phone number, service of interest, preferred language, preferred contact time, and an optional general message. The site also stores your privacy preferences in your browser.
Why we collect it
We use this information to respond to your inquiry, understand which service you are asking about, contact you at an appropriate time, and manage administrative follow-up. We do not sell your information to data brokers or use it for purposes unrelated to operating the practice.
Where it is stored
Website inquiries are transmitted to and stored in the practice's access-controlled application database. A notification containing the inquiry details may also be sent to authorized practice email inboxes so staff can respond. This public website does not process payments or serve as the practice's clinical record or telehealth platform.
Who can access it
Access is limited to authorized practice staff who need the information to review, assign, and respond to inquiries. Staff access requires authentication and is controlled according to assigned responsibilities.
How long we retain it
Administrative inquiries are retained while reasonably needed to respond, manage follow-up, maintain appropriate business records, and meet applicable legal obligations. A fixed automatic deletion schedule is not currently implemented. You may contact info@serendipiatherapy.com with a privacy or deletion request, subject to any records the practice must retain.
Information you should not submit
Do not use these forms to send detailed mental health history, diagnoses, medication information, trauma history, medical records, payment information, or identifying information about another person. The forms are not a secure clinical messaging channel. If the practice may be a fit, staff will direct you to the designated clinical system for intake and sensitive information.
Security and service providers
We take reasonable measures to protect website inquiries, but no internet transmission is completely secure. We use commercial service providers to host and operate the website and application database. Formal clinical information is handled separately in the practice's designated clinical systems.
Your privacy rights
Depending on where you live and applicable law, you may request access to, correction of, or deletion of administrative information you submitted through this site. Contact info@serendipiatherapy.com to make a request. Separate clinical records may be subject to different legal retention requirements.
